Why join the Lifestyle Change Program at UW Health?

This program is a year-long commitment, starting with weekly classes for the first 3 months, then biweekly and monthly for the last 8 months. The group class meets virtually or in-person at:

Classes are available Monday through Wednesday between the hours of 10 a.m. and 6:30 p.m.

By enrolling in the Lifestyle Change Program, participants will:

  • Connect with a certified Lifestyle Change Coach

  • Learn realistic strategies for improving everyday food choices, physical activity, sleep and well-being

  • Join a support network with others on a path to better health

  • Receive a free Dane County YMCA membership

The program is often covered by insurance. Please contact your insurance provider and ask about coverage for the National Diabetes Prevention Program (code G9886 or G9887).

We invite partners, spouses, or support persons to enroll together, as this can provide support and accountability that fosters success in the program.
